Educational Workshop 6.5 Hours
 Achieving Person Centered Care and Culture
Change requires teamwork. It is
recommended that each facility train a Person
Centered Care Team to implement the
Communication Programs social model for dementia
as well as administer and utilize data from the
Reality Comprehension Clock Test.
Social Model for Dementia
- The Communication Programs
contains a brain stimulating
curriculum.
- Each exercise is designed to
MATCH the elderly memory
impaired person’s cognitive
skills and abilities.
- Communication Programs are a
vital part of this social model
for dementia.
- Each student will learn the
components of Communication
Programs and recognize how to
determine which level of
Communication Programs is
appropriate for each memory
impaired participant.
- Each student will identify
how the brain stimulating
exercises are incorporated into
each participants care plan.

Recommended Team Members:
Administrator, Director of Nursing, MDS Nurse,
Restorative Nurse, Social Worker, Recreational
Therapist, Activity Professional, Marketing
Director. Additional staff may include
Rehab Director, Occupational Therapists, Speech
Therapists, L P N ‘s, C N A’s, Dietary Director,
Environmental Director.
The Communication Programs Workshop also
includes Reality Comprehension Clock Test
educational training. At the completion of
this workshop students will be certified to
administer the RCCT as well gain knowledge of
how to implement the Communication Programs
social model for dementia.
